Programs
COPS/Linkages Program
Provides assistance to people with high level, complex support requirements. We also assist people to identify needs and the options available to them. COPS/Linkages may purchase a full range of in home support and community access services in consultation with the person and their family. The program provides service co-ordination, service monitoring and case management, where identified as necessary.
Respite Links
Provides medium to long-term support for people diagnosed with dementia and their families and friends. This program is particularly directed towards people supporting someone whose behaviours may be described as challenging. As a consequence these individuals may experience difficulty in accessing mainstream respite services. Service co-ordination, case management and purchase of services are available.
Continuity of Care Program
Provides support on discharge from hospital for up to 8 weeks. Services offered include, but are not limited to: personal care, assistance with housework, shopping, meal preparation, respite care and home modifications.
Domestic Assistance Program
Provides on-going domestic assistance for those people who require basic house cleaning support.
Commonwealth Respite for Carers Program
Provides short to medium term respite care services and is directed towards individuals and families with complex and special support needs, which are unable to be met by existing services.
Community Support Options Program
Provides support that will assist people to develop skills and increase their confidence and ability to participate and be involved in community activities of their choice.
Individual Support Packages
Individual Support Packages (ISP’s) or other individualised funding arrangements, are managed on behalf of clients and provide the administration support required. Service co-ordination and case management services are provided if necessary.
Transitional Support Program
The Transitional Support Program provides integrated support services to people in need of assistance as a result of illness and following discharge from hospital. The aim of the program is to assist individuals and their families who are “at risk” as a result of hospital admission and who require support services. This program can provide support for a maximum of 12 weeks.
Flexible Family Support Program
The primary goal of family support is to provide a wide array of options to families of people with disabilities and older people that enable them to stay together and to be welcomed, contributing members of their communities. This is done through developing the families’ natural capacity to meet the needs of family members; enhancing the capacity of communities to value and support people with disabilities and their families; and offering additional supports such as staff, goods and services and financial resources.
